Clinical Case Manager - Irving, TX

CNS Texas facility currently has an opening for a Clinical Case Manager. The position is responsible for the development, implementation and management of all clinical aspects of CNS. The six core components of case management include: processes and relationships, health care management, community resources and support, service delivery, psychosocial intervention, and rehabililtation case management. The essential activities of the position include the following:

. collect/assess in-depth info about the client function to identify needs
. Plan specific objectives, goals and actions designed to meet client needs
. Implement and intervene in order to accomplish goals
. Monitor and evaluate plan effectiveness making changes as required
. Supervise and directs all aspects of program
. Participate in program client program reviews and conferences
. Produce reports and documentation

The position is full time, Monday-Friday, with some on-call and overtime work.

Knowledge, Skills, and Abilities Required:

. Requires Bachelor's degree in a related field. Masters preferred.
. Minimum of 5 years experience in rehabilitation. CCM preferred.
. Demonstrate a capacity for and interest to absorb a global knowledge of medical and clinical rehabilitation aspects relating to traumatic brain injury.
. Experience in which supervisory abilities have been demonstrated.
. High level of accuracy and dependability in decision making, coordination of client programs, documentation, and ability to evaluate situations as they occur.

Nurse [PM Shift] - Irving, TX

CNS Irving, Texas residential facility currently has an opening for a PM Shift Staff Nurse. The position is full time Monday - Friday, 3:00 PM - 11:30 PM, no weekends! As part of the nursing team, you will perform nursing techniques for the comfort and well-being of the patient. Administer or supervise the administration of prescribed medications and treatments. Take vital signs, including temperature, pulse, respiratory rate, and blood pressure regularly to detect deviations from normal and monitors the medical progress of patients, double check and follow physician's orders and individual nursing care plans, including assisting patients with morning routines, catheter irrigating and assisting with respiratory treatments. Requires basic computer skills to complete conferences, make charting entries, communicate openly and clearly with associated staff at the clinic and residential site, formulate specific programs to outline particular patients needs in a format comprehended and utilized by the residential and clinic staff. In coordination with other nursing staff, supervises, assists with and delegates the distribution of medications to the appropriate residence of each patient.
