Patient Evaluation Criteria
1. Was the evaluation performed by clinical versus marketing staff?

2. Did the evaluation include a thorough review of past medical/rehabilitative
care and treatment? Yes No
3. Did the evaluator spend a reasonable amount of time with the client to truly
understand his/her needs? Yes No
4. Did the evaluation include a detailed projection of rehabilitation program
cost and outcome goals? Yes No
5. If not, is this available?
Yes No
6. Did the evaluation include a projected discharge living status, i.e.,
independent, group home, nursing home, parent's home?
Yes No
7. Did the evaluation list a length of time the rehabilitation program would take
to accomplish the goals stated in the evaluation?
Yes No
8. Did the evaluation include objective, quantifiable goals for the program to
be evaluated against? Yes No
9. Did the evaluation provide you with more information than you had before
the evaluation? Yes No
